Before you start - check this out!

This form asks for your preferences, such as memory quilt layouts and quilting design/style. To ensure you don't lose any information you have entered, please go to my Gallery of work for layout examples, and my Available Quilting Designs (if scheduling a memory/t-shirt quilt, filter the available designs by "t-shirt quilt options" to see designs that work great on t-shirt quilts!) before you start the form.
